Thank you for trusting inSite Digestive Health Care with your health. We look forward to meeting you and helping you with all of your GI needs. We have put together a guide to help you navigate through our new patient registration process.

 

We want to make the scheduling process easy and convenient for our patients. You can now request an appointment online.

You may also call the nearest office and schedule with them directly. For a list of offices near you, please use our Office Locator Tool.

We encourage you to register and start using our Patient Portal to complete your health history and maintain communication with your provider. You will receive an email with a PIN for registration if you’ve provided your email address to us. Please follow the instructions within the email to register and complete your health summary prior to your appointment. If you did not receive a PIN via email, call our portal help desk at 866.630.0612 and press 1 or send us an email at [email protected]

We continue to look for ways to make your health care experience as hassle-free as possible. To minimize paperwork associated with your visit, please print, complete and sign the forms and consents below before your appointment. You can return these forms to our Patient Information Center by:

In Person:
Although we encourage you to have your forms completed and sent to us prior to your appointment, you still have the option of walking them in to the office.

By completing the forms prior to your appointment, you allow the doctor to focus on addressing your current medical conditions.

  • Please arrive 15-20 minutes prior to your appointment time to fill out or process appropriate paperwork.
  • Please bring the following items on the day of the appointment:
    • Insurance cards
    • Photo ID
    • Form of payment
    • Any necessary authorizations or referrals from your insurance provider
    • Completed forms
    • Copies of your recent test results (i.e. blood work, x-rays, CT scans, emergency room visit summaries and/or surgery reports

Please note, we may request more information or a confirmation of the information we have on file at the time of the visit  to ensure that it is up to date.
